That'll be for the 90 minutes only market.

Yup - if it's tied after 90 minutes then there are 30 minutes of extra time. If it' s tied after that then it's a penalty shootout.
Most markets will be for the regulation 90 minutes. Betfair have a "qualify for the next round" market in soccer knockout competitions - perhaps other books will do this when all the fixtures are finalised.

If the game is tied after 90 minutes, is the 30 minute extra time sudden dealth or do they play the entire 30 minutes?

I would think sudden death, if not, that's lame.

No sudden death guys.
This rule was experimented in the 1996 European Championship and modified some time after that - if there is a goal in the first 15 mins of extra time the game ends after it expires, 2nd 15 mins are not played, and it was considered unappropriate for soccer, which is my opinion too.

Penalties are not so cool, but it's funny to watch guys like Palermo (Argentina) missing three penalties on the same game.
Or Roberto Baggio (Italy) missing one to give Brazil the championship...

Just to clarify, this is the order ;
- 90 minute result, but if tied;
- 30 minutes extra time (two 15 minute halves) played in full, if still tied;
- penalties (5 each then sudden death after each side has taken five each ... penalties stop when the other side cant match them ... i.e. after 4 penalties each it is 4-2 to one team theres no point carrying on ... like the ninth innings in baseball I suppose)
So, a game finishes 2-2 after 90 mins
Extra time each side scores once so 3-3 a.e.t
Penalties, team scores its first three other team misses its first three. Game over
All betting markets are nearly always for 90 minutes only. Some markets ammend this for "To win" which means after everything that night.
Live betting can sometimes offer extra time odds.
Sorry for spelling it out but I remember how confusing baseball was when i started.

1. I'll have to hijack espn's signal first and then put some good narrators there.
2. Then, I'd send Bruce Arena to fight in Iran.
3. Then I'd buy Ronaldo and Ronaldinho and rename them to Ronald James and Ronald James Jr.
(Although not necessary 80% of US team players have foreigner names: Cherundolo, Mastroeni, Bocanegra, Reyna, etc.)
When those 3 steps are done, maybe i'll be able to start advertising football in USA.
