If someone is clearly suffering from depression (fits all the symptoms, sad all the time, etc), but they are too afraid or unwilling to go to a therapist / get on medication, what else is there you can do for them??

Every situation is different, but this is pretty good advice to start. Depending on the person, it may or may not work. Often people just get into a rut or a slump and need to get out of the funk. Getting them out and just get them going again is the best solution. If this doesn't work, then maybe meds or counselling are the next step.
I'm not a fan of self help books any more than I'm a fan of the latest excercise gizmo. They are about the same, they aren't offering anything that you couldn't do by yourself, and in both cases the book or gizmo will be left unused after a few weeks with no real affect gained.
I'm also not a fan of counselling. I think there's too many piss poor counsellors out there. Often a person can end up worse than they started if a counsellor gets them started in a bad direction. I've never known anyone that's gone through counselling and made any remarkable progress with any kind of problem. But, I've known a few that felt they were just wasting their time at best, or getting bullshit advice at worst.
GL with this case, depression can be a serious problem especially if left untreated.
