Since everyone else has ignored your second question, I will answer it.

The rollover is for your deposit + bonus.

SO you deposit $2,000 and recieve a $500 bonus which brings you to $2,500.

You will need to do $2,500 x 5 = $12,500 in rollover to be able to withdrawal.

That is how most rollovers work in the industry.
