It does happen. The best example I can think of is when the Ravens played the Bengals early this year, they had first & goal late in the game... they scored, there was a penalty. then there was an offensive pass interference call which looked totally bogus, so another penalty. then they called defensive holding on the Bengals the very next play to give them another shot (call looked even more bogus), and they still couldn't score.
