    well, i've never filled out anything at any of these books:

    pinnacle
    cris
    greek
    wsex
    matchbook
    five dimes
    betjm
    legendz

    basically every book out there. though i guess it depends on how you deposit. stay away from ************.

    well, unless you are doing something illegal, it probably takes less time to fill out the form than to post in this thread

    i don't think any of the sportsbook.com books require any forms. they don't seem to be paying anyone either though. i'd stay away.

    Find a book you like and email them and ask how you can deposit. www.betjamaica.com would probably be a good choice.

    Where are you located?

    In Europe:

    Betinternet
    Ladbrokes
    Paddypower

    Keep all deposits to a limit and you'll be okay.
